I'm trying to decide between this product an Survey Monkey. Can I create a quiz with 12 multiple choice questions that uses buttons, is NOT self grading, allows media (photos only) and gives users the opportunity to submit their email address for results and to know if they have won a prize?

Did you decide which to use? Survey Monkey is for surveying while PP does so much more. I use SM for surveys but for any graphics questions, etc I use PP every time with success. It can do all you want. Need more details?
