We had to leave the doctor voluntarily due to medical negligence; discrimination and office polices of Dr. Sunil modi to my 90 year old mother.

Update ( 06/13/2023): Please provide the confirmation of letter recd by us of dismissal

I provided the office 4 weeks to provide documentation ( confirmation of receipt)

We did not receive any information.

Here is an update

To leave your 90 year old hospice patient with no care undercover, he dismissed care of primary care giver ( myself) with no intimation is gross negligence.

If the primary care giver is dismissed you knew they would get handicapped?

You played games with two lives. You are giving excuses of patient having issues with doctor? What are the issues with doctor? We never had any issues till all this happened

Was it discrimination?

Your both old patients ended up in the hospital.

Our suggestion please stay honest.

This is a highly unprofessional Buisiness. They have no compassions towards senior citizens too.
Dr. Modi is the person this is in reference too
