B&H has great products and provides a truly great service. I had an issue where it was charged to the wrong card and they resolved the issue in enough time to still receive the gear on the initial purchase date.
My wife and I ordered a lot of camera/video equipment from then in the past week. They got part of my wife's order wrong. When I called them about it they said we could either ship it back and after they receive it and inspect it (3-5 days), they would ship us a replacement. The other choice we had is to pay for a new one and ship the old one back. If we had ordered the wrong thing then I could see these options. We placed the order correctly and they made the mistake. They Arne't willing to fix their mistakes. They want the customer to pay for their mistakes. We will never shop here again.
I have purchased all my cameras and video equipment from this store. Besides camera and video they have computers, audio, lighting, binoculars on consumer and professional level. They are on top of the game for equipment authorized to be sold in the USA. Excellent costumer service. The return policy is standard.
I have shopped B&H for a long time. In the 90s when I lived in Philadelphia, through now in Reno, NV. I only had one problem years ago, and it was handled in a professional and courteous manner. Can't really say there is an equal.
Consistently the best prices, with free shipping (at least for the kinds of things I usually order), helpful and knowledgeable customer service, and at no additional cost for tax using Payboo. Can't be beat.

Basically i love B&H but they are offline too many times for holidays or events or something. I can understand their bricks and mortar location but their whorehouse and online systems should be 24-7. I can't tell you how many thousands of dollars from me alone i sent Amazon instead of B&H due to their systems being offline for some holiday or what ever. No disrespect but its online and should be 24/7. The other crappy part is their gift cards and credits. It should be attached to the acco8unt like Amazon because I've found by credits in the spam folder and if i didn't it would be gone forever. Also you can't paste and copy the long code you must type it. When you have 15 or 20 of them it sucks.
I've been a customer of B&H for over 20 years and I've never been disappointed by their used items for sale. Items always seem to be better than they are rated and if I've ever had a problem, I can easily return the item.
I am a repeat customer of B&H for several reasons, one of which is how easy it is to find what I need on their website and then pay for it. I also rely heavily on the customer ratings and the description and details that are provided for each item. Last, it's good to know that B&H prices are competitive.
Answer: ANy refurbished item we sell has been refurbished by the manufacturer or official US distributor, Nikon USA or Canon USA etc. Other used items are each rated on a numeric scale and we tend to be conservative so customers are not disappointed.

It is industry-standard practice for a retailer to receive a customer's return before shipping a different (or another) item. The standard alternate option is a return-for-refund while the customer simply places a new order for the item originally wanted. I am sorry neither of these options was satisfactory. In particular circumstances we can advance ship the new item as soon as we can confirm the item to be returned is en route to us.
